In your example Tolas and similar cards would trigger in step 3, while the card is still in play, which cannot happen. Or, step 3 would resolve while the card is "in the air" between play and the discard pile, which I also don't think is the case.

I haven't thought through all the details, but an "in the air" step might be a cleaner solution than errata to add "instead of".

Well I honestly don't think we need any errata, there isn't anything wrong with the cards themselves. And IMHO there's nothing overly problematic with the RAW either. At least nothing wrong or contradictory - only insufficiently clear. Which, along with our presumptions, can lead us to incorrect conclusions.

The simplest solution would be just to modify the line about destroyed creatures in the glossary with something like: "When a card is destroyed, it is placed in its owner’s discard pile unless instructed otherwise by card effects."

or to be pedantic, add a line like this:

"If there is a constant ability referencing the destroyed creature (example: “after an enemy creature is destroyed fighting this creature...”), when the destroyed creature leaves play, it is placed in the instructed out-of-play area instead of the discard pile."

And that's it. All problems solved. And as a matter of fact, we actually do know this is how the game works, as shown by the rulings they made so far. So to make life simpler for us, we might as well pretend as if this line is already in the rulebook.
